I request that a copy of the following documents be provided to me: 1) Any DoD regulation requiring DoD military, civilian personnel and/or contractor employees to sign a form certifying that they are willing to undergo a specific-issue polygraph examination if classified information they had access to has been leaked as a condition for access to Top Secret, SCI or higher information. 2) Any such certification form adopted pursuant to such regulation. DoD referred to such a program in its polygraph program report to Congress for Fiscal Year 1999: In an effort to eliminate or reduce the number of unauthorized disclosures of classified information to the media, we plan to implement a new policy. As a condition for access to Top Secret, SCI or higher information, DoD military, civilian personnel and contractor employees will sign a form certifying that they are willing to undergo a specific issue polygraph examination if classified information they had access to has been leaked. We believe this will serve as a deterrent to individuals who may be considering leaking classified information. In order to help you determine my status for the purpose of assessing fees, you should know that this request is made for public awareness purposes and not for commercial use. AntiPolygraph.org is a grassroots organization of freely associating individuals and is not a profit-seeking enterprise. I am willing to pay fees for this request up to a maximum of $25. If you estimate that the fees will exceed this limit, please inform me first. I request, however, a waiver of fees for this request because disclosure of the requested information to me is in the public interest because it is likely to contribute significantly to public understanding of the operations and activities of the DoD and is not in my commercial interest.
